Application For Site And CF

​Introduction

This application is available at the OSC counter.  It only involves the development of which was given approval before April 2007. There are two levels of CFO application: 

  1. Site Visit Application

    • Applicants must submit two sets of each document to be submitted to the technical agencies and to be filed as OSC reference. 

    • The  technical agencies involve are the Building Department, Planning, Engineering and Park & Recreation as well as external technical agencies such as the fire brigade, IWK and syabas​.

  2. Form E Application ​

    • Applicants must submit two sets of each document to be submitted to the technical agencies and to be filed as OSC reference. 

    • The   technical agencies involved are the Department of Building and OSC. Download

Method of Payment

    Payment must be issued in two sepa​rate  cheques / bank draft bearing:

    • MAJLIS BANDARAYA SHAH ALAM - for processing fee.
    • MBSA – AKAUN CAGARAN - for deposit.​​​
